SDTMA FTO Program Information request

To All,
Merry Christmas, hope you are all enjoying the holiday season.
We are going to be revamping our FTO Program over the course of the next several months and I'm looking for information on other agency's programs.
1 - What Lenis the length of your training program for new officers\recruits and for lateral officers?
2 - What does that program (structure/schedule) look like?  2 weeks orientation, 4 weeks patrol, 1 week traffic, 1, week investigations etc.
3 - Can I get an electronic copy of your templates for Daily, Weekly, Bi-weekly and Monthly Evaluations?
4 - Can I get an electronic copy of your FTO Manual?
5 - Do any of your agencies use a software program to manage your evaluations and FTO database?  If yes, what and do you like it?
